Dance Workshop

1. Voluntary Participation & Assumption of Risk

I understand that participation in this dance workshop involves physical activity, including but not limited to dancing, stretching, jumping, and other movements. I voluntarily accept and assume all risks of injury, illness, or property damage that may result from participation, whether or not caused by the negligence of Bay Area Youth Arts Foundation (BAYA), its officers, directors, staff, volunteers, or instructors.

2. Release of Liability

To the fullest extent permitted by law, I hereby release, waive, and forever discharge Bay Area Youth Arts Foundation, its officers, directors, employees, contractors, volunteers, instructors, and agents (collectively "BAYA") from any and all claims, demands, damages, costs, and liabilities arising out of or in connection with participation in this workshop, including claims for personal injury (including death), illness, property damage, or any other loss, whether or not caused by the negligence of BAYA.

3. Physical Fitness & Medical Condition

I confirm that the participant is in adequate physical condition to engage in dance activities and is not aware of any physical limitation, disability, or medical condition that would prevent safe participation. I agree to immediately notify BAYA staff of any relevant health concerns before the workshop begins.

4. Medical Authorization

In the event of an accident, injury, or medical emergency, I authorize BAYA staff to seek or provide emergency medical assistance on behalf of the participant. I understand that BAYA is not responsible for any medical expenses incurred as a result.

5. Facility Rules & Conduct

I agree to comply with all rules and instructions of the dance studio and BAYA instructors. I acknowledge that BAYA reserves the right to remove any participant from the workshop for behavior deemed unsafe or disruptive, without refund.
